A couple of weeks ago we began hearing about Mafia 4, which is reportedly being built in Unreal Engine 5. That report detailed a new Mafia's place within the series — expected to be a prequel — and now, new details from XboxEra have added a bit more context.
One of the podcast's hosts, Shpeshal_Nick, has gathered some information on the next Mafia game. Nick says that he's been told it'll be set in Sicily in the late 1800's, early 1900's, and the game will "follow the family of Don Salieri", a main character in the first Mafia game.
The host also says that the prequel is set to be a little less open than Mafia 3, following a structure more closely aligned with the first two games in the series. Here's the timestamp of the show's discussion on Mafia 4:
Both the initial Kotaku report, and this added context from XboxEra aren't official confirmation by any means, and we're still waiting on 2K and Hanger 13 to officially announce another Mafia game. Even so, here's hoping confirmation comes soon!

Hmmmmm. I’d have liked a New York setting or a vegas setting and larger open world than 1. Combining the best parts of the first three but with more of a 1 setting but larger scale. Steal elements from the Godfather game.
There was nothing wrong with 3 very good game just not classic Mafia setting for me.
Or EA could just give us a remake/remix of Godfather game with some new elements.
Or that Scarface 2 game that got cancelled
Or all three would be nice!
Sounds awesome. Feels a bit like those scenes from Godfather/Don Corleone’s origins stories.
I can't wait. I always enjoyed the Mafia series. Even 3 even though it wasn't on the same level as 1 or 2.
