I'm going to go with the games that impacted me most. I could list 100 games that I think are amazing and worth playing, but I'm going to go with the ones that really made me stop and love this hobby. In biographical order!
1) Commander Keen IV: Secret of the Oracle. It came with the first gamepad I ever bought for PC. It's still great to me.
2) Half Life. Blew me away. I had other shooters before but none of them held a candle to this. And at the time the graphics were amazing. The Counterstrike mod may still catch on
3) Diablo 2. Clicked around this game and the expansion for countless hours with all the characters. Still fun.
4) Super Mario Sunshine. I got a Gamecube for Christmas and quickly got this game. I actually think it was treated unfairly at the time. It holds up really well and is still a blast to play.
5) Legend of Zelda Gamecube disc which included all of the console releases from the NES, to the N64 and the demo for Wind Waker on Gamecube. Yes, this is a cheat because it is more than one game but it came with the system on one disc and was awesome. Been a Legend of Zelda fan ever since.
5. Starcraft, thou the atmosphere and gameplay were (too) addictive for my poor RL =)
4. Warcraft 3, my intro to WoW - a nasty friend got me hooked, and then...
3. Age if Conan, I like it's complicated combat, fatalities and especially the tutorial levels.

These are based on the impact and enjoyment I got out of them at the time, not how I would necessarily view them if I played them again today. In no particular order:
Ultima IV (Commodore 64) - Probably the first game where I got totally immersed/lost in a fantasy world for weeks on end.
Witcher 3 (Xbox One) - Blown away by the staggeringly large world and amount of stuff to do and sheer quality of it all.
Alone in the Dark (PC) - First time a computer game actually scared the crap out of me, time after time.
GTA 5 (360) - This game just seemed to offer it all, racing, flying, sailing, shooting, fighting, exploration, compelling story, amazing graphics, music, sounds, attention to detail, you name it.
Tomb Raider (PS1) - First fully realised 3D world I'd experienced that you could fully explore by running, shooting, climbing, swimming and get chased by a T-Rex! Possibly the biggest jump in gaming 'progress' for me personally.
As @stylon these are based on enjoyment/gaming impact and in no particular order
Mario Kart (SNES) Me and my brother spent months on this and could do Ghost Valley in just under 15 seconds a lap.
Dark Souls series I haven't played a series I found so hard yet so compelling to play on.
Halo CE The game that got me into Xbox
Gran Tourismo The game that got me into console gaming
Alien Isolation I have never felt terror and excitement like it and I think its better than the first film! The whole feel of this game is beyond compare for me, can't rate it highly enough!
An honourable mention has to go to R-Type as I have played and owned this game on Spectrum, Amiga, Xbox360 and due to B/C my Xbox One!
Top 5 of all time...hmm. In no particular order:
Unreal Tournament - my all time favourite PC game.
Batman Arkham City - I spent ages playing into the small hours after getting for Christmas one year.
Gears of War 2 - Bought this knowing nothing about the series, based on Amazon reviews. Loved it ever since.
Carmageddon - Insane violence loosely associated with car racing. I'm excited to see the Xbox reboot.
PGR4 - the game that converted me fro PS to Xbox.
